GP69 form is required when applying for registration by the Teachers Service Commission, TSC. The form is part of the mandatory documents that an applicant must fill, scan and attach together with other documents during the TSC number application process.
The form is filled by a qualified medical officer (at a Public Health facility). The applicant is subjected to a number of medical tests before the medical officer gives his/ her final report. The form contains the applicant’s basic information, name and counter signing by the medical officer and the medical report of the applicant.
